Admission Process

Admission instructions from the hospital, focused on accurate registration, required IDs, deposits, and special document needs.

1

Share the patient's name, age, sex, religion, address, contact number, and other relevant details carefully at the time of admission.

2

Submit an Aadhaar card, mediclaim copy, or another government-issued ID during admission.

3

For maternity admissions, submit Aadhaar cards for both husband and wife. If post-marriage Aadhaar details are not updated, keep an affidavit or marriage certificate ready.

4

Pay the required deposit for admission. Interim bills may be issued when hospital costs exceed the deposited amount.

Documents to keep ready

  • Aadhaar card or other government-issued ID
  • Mediclaim copy if applicable
  • Old doctor papers and previous investigation reports, if available
  • Insurance authorisation letter, if received
  • Original company authorisation note for corporate insurance
  • Passport and visa copies for international patients
  • Marriage certificate or affidavit for maternity cases where Aadhaar records are not updated

Check details before admission is completed

The PPT states that changes may not be possible once admission is done, so patient and relative details should be checked carefully before submission.
